• Valid U.S. government credentials
• Statement, including viable contact information, presented on government/department letterhead.

Please note that both veteran/retired and active service personnel are eligible for this program. 

A Service Personnel Program Representative will confirm or deny your application once we have received the form.

Items will not ship until your application is approved, we have contacted you, and payment is received.
